          <jats:p>Background: End-stage renal disease (ESRD) and chronic kidney disease (CKD) are growing more common.Whereas hemodialysis (HD) is the most common kidney replacement therapy for individuals with diabetes, dyslipidemia, anemia, and bone mineral abnormalities.&#13;
Objective:  The objective of this study were to asses prescribing patterns in stage 5 of chronic kidney disease patients undergoing Hemodialysis.  &#13;
Method: This study was an observational retrospective cohort study, it was conducted at Owaisi hospital, between march and April 2026. All adults on chronic HD were included. All information was were collected from governmental electronic health record, and patients were asked to answer some questions when data was not found in the system.&#13;
Result: During the study of 50 patients with Chronic Kidney Disease Stage V, patients were mostly found male, Having 62 percent and 38 Percent were female.The number of drugs per patient ranged from 2 to 15, with a mean of 7.74+SD. A lot of the patients were between 51 and 60 years old, which is 42 percent. Some patients were 41 to 50 years old, which is 26 percent and patients who were 61 to 70 years old which is 24 percent. The most commonly used drugs are Furosemide (80%), Sevelamer (60%), Sodium Bicarbonate (60%), Inj. Erythropoietin (20%), Inj. Sucrose (20%) and Pentaprazole (40%). When we looked at the patients in the study we found that high blood pressure was the common health problem that these patients had. In fact 76 percent of the patients had blood pressure. A lot of the patients were having 36 percent both blood pressure and diabetes. Some patients also had anemia, which's 16 percent and some had hypothyroidism, which is 4 percent. A few patients had artery disease, which is 2 percent. All of the patients we surveyed had more than one health problem, and the most common combination was high blood pressure and diabetes, which is Chronic Kidney Disease Stage V patients, with high blood pressure and diabetes.&#13;
Conclusion: In addition to widespread polypharmacy—the majority of patients were taking six to nine medications, This study of 50 CKD Stage V patients showed a significant frequency of diabetes and hypertension as comorbidities. The results emphasise how difficult it is to manage end-stage renal disease (ESRD) and how effective medication therapy is necessary to minimize interactions and enhance results. The course of the disease may be slowed by early management of DM and HPT.</jats:p>
